What goes wrong is rarely
the pouch. It is the skin.
On a urinary stoma, what decides how the day goes is not capacity — it is keeping the skin dry around the stoma. That skin does not do well staying damp, and when it is not dry, adhesive does not hold as it should. Urine gets to it by two routes. One is backflow: you lie down, the pouch tilts, and what had collected finds the stoma again. The other is creasing: a round, rigid baseplate on an abdomen that is not flat throws folds — and every fold is a channel.
Two answers, one for each route
Backflow is answered by the anti-reflux valve: a one-way valve inside the pouch that lets urine down and does not let it back. You lie down, turn over, bend to tie a shoe — what has collected stays at the bottom, not against the stoma. Creasing is answered by the flower-shaped hydrocolloid baseplate: instead of one rigid disc, petals that each follow the shape of the body on their own, finished with a soft non-woven border that seals the edge down. Fewer folds, fewer channels. These two details are the ones that tell over time — not on day one.
Which stoma it suits — and when it does not
What decides this is not the diagnosis but the shape of the stoma. The baseplate here is flat: it needs a stoma that protrudes at least slightly above the skin. If yours is flush with the skin or retracted, the right product is the convex one, not this.

In practice

Measure
Measure the stoma with the guide in the box and cut the baseplate 2–3 mm larger than its diameter. Measure again every week for six to eight weeks after surgery: the stoma shrinks, and an opening that was right in the first month leaves skin exposed in the second.

Prepare the skin
Clean around the stoma with water or a mild cleanser and dry it completely. No oily creams or lotions — adhesive will not hold on them. Hair is trimmed, not shaved.

Apply
Peel off the backing paper, centre the baseplate on the stoma and press from the bottom upwards. Smooth the petals down one at a time and hold your palm over it for 30 seconds: body heat helps the adhesive bond.

Empty
Drain when the pouch is a third to half full, not when it is full — it is the weight that lifts the baseplate. Open the tap, empty, wipe the outlet, close and clip the cap.

At night
Fit the connector, attach the night bag before you sleep and keep it below the level of the stoma, never up on the bed. In the morning disconnect, rinse and let the connector dry.

The anti-reflux valve
Inside the pouch there is a one-way valve: urine passes down and does not come back. B.Braun states it plainly — anti-reflux valve to avoid urine backflow. It matters most when you are not upright: in bed, on the sofa, bending to tie a shoe. That is exactly where a pouch without a valve puts urine back against the stoma.

The flower-shaped baseplate
The hydrocolloid protector is not a round disc but a set of petals. Each petal sits on its own, so the baseplate follows the curves of the body instead of bridging them with folds. The edge is finished with a soft non-woven border that beds the rim down onto the skin — and the rim is where almost every leak begins.

The tap, and its cap
Emptying is done through a B. Braun tap with marked open and close positions and ergonomic grips — you never take the pouch off to drain it. The end cap adds a second layer of security in case the tap is nudged open inside your clothing.

“I have just come out of hospital.”
Start with transparent. In the first weeks the stoma is being watched, and a clear film saves you removals you did not need. Ask for a free sample before you commit to a month's box.

“I wake three times a night.”
Then you do not need a bigger pouch, you need a night drainage bag. It attaches with the connector already in the box — one per pouch — and empties for you all night. The anti-reflux valve is what makes lying down safe.

“Will it smell?”
The film is odour-proof: while the pouch is closed, nothing gets out. Smell exists only during emptying, which is why you wipe the outlet before you close the tap and clip the cap.

The beige version has a soft non-woven outer and does not catch the light. The real trick is not the colour though, it is when you empty it: drain at a third full and the pouch stays flat.

“Will I swim again?”
Yes. Empty it first, check the cap is clipped, and when you come out rinse with fresh water and pat dry. After a long time in the water it is safer to change the pouch. Ask your nurse before the first time.

That is what the cut-to-fit baseplate is for. Measure with the guide and cut 2–3 mm larger than the diameter. Then measure again: a stoma keeps shrinking for six to eight weeks after surgery.

The same range, the other versions
Flexima® Uro Silk comes in two colours and with a flat or a convex baseplate. Your stoma — not your preference — decides which.
It is a one-way valve inside the pouch. Urine passes from the stoma down into the pouch and cannot travel back up. The difference shows the moment you stop being upright: in bed, on the sofa, bending down. Without a valve, what has collected slides back towards the stoma and sits against the skin around it — and the skin around the stoma is what comes under strain first on a urostomy. There is nothing to operate: the valve is inside the pouch and works on its own.
Beige or transparent — which should I get?
It is the same pouch: the same cut-to-fit baseplate at 12–55 mm, the same anti-reflux valve, the same tap, the same rotating connector. Only the film changes. The practical rule: transparent while somebody needs to see — the first weeks, check-up appointments, or any time you are asked to watch the colour of the urine — and beige for ordinary life, because the non-woven outer does not outline itself or catch the light under clothing. Plenty of people keep both in the cupboard.
How often do I change the pouch?
A one-piece system is changed whole every time — usually once a day, possibly every other day depending on your skin and how long the adhesive holds. That is why the box holds 30 pouches: it covers a month at one change a day. The exact frequency is set by your stoma nurse or doctor — do not stretch it because “it is still holding”, and do not stretch it to save money either, because the skin pays for both.
Can I shower with the pouch on?
Yes. The pouch stays in place in the shower. Two practical points: do not aim hot water straight at the baseplate, and pat dry rather than rub — a non-woven outer holds moisture a little longer than a plain transparent film. The easiest routine is to pair showering with the change: shower, dry the skin thoroughly, and put a fresh pouch onto a dry surface.
What happens at night?
You do not need a bigger pouch — you need a night drainage bag. It attaches with the universal connector already in the box, one per pouch. The night bag sits below the level of the stoma — on a stand on the floor, never up on the bed. The connector rotates, so when you turn over the tubing follows instead of kinking, and the anti-reflux valve keeps urine away from the stoma for as long as you are lying down. In the morning you disconnect, rinse and let it dry.
My stoma is flush with the skin. Will this work?
Probably not, and it is better to know that before you order a month's box. This baseplate is flat and needs a stoma that protrudes at least a little; on a flush or retracted stoma urine readily finds its way under the adhesive and the baseplate tends to lift. The answer is a convex baseplate, which presses gently around the stoma and brings it forward, often together with a support belt. Send us a photograph or talk to our stoma nurse before you decide.
How do I compare this pouch with another brand?
With six questions, the same ones for every brand. 1) Does it have an anti-reflux valve? If the manufacturer does not say so, assume not. 2) What shape is the hydrocolloid baseplate — one rigid disc, or petals that follow the abdomen? 3) How many connectors are in the box? Here there are 30, one per pouch; if they are sold separately, the real monthly price is not the one on the label. 4) How much cutting range does a single reference cover? 12–55 mm means you do not change reference while the stoma shrinks. 5) Is the same size available in both beige and transparent? 6) Is it latex- and phthalate-free?
