Stoma Leakage: Causes, Prevention and Practical Solutions

Stoma leakage is one of the most common concerns for people living with a stoma. Beyond the practical inconvenience, it can affect confidence and everyday comfort. In most cases, however, leakage has specific causes and can be managed effectively.

What is stoma leakage?

Leakage occurs when stoma output passes underneath the baseplate instead of flowing directly into the pouch. This may lead to skin irritation, discomfort and more frequent appliance changes.

Common causes of stoma leakage

1. Incorrect opening size

The baseplate opening should fit as closely as possible around the stoma. If the opening is too large, output may come into contact with the surrounding skin.

2. Flat or slightly retracted stoma

If the stoma does not protrude sufficiently, output may flow underneath the baseplate. In such cases, a convex baseplate may improve sealing and reduce leakage.

You can read more about when convex stoma pouches are recommended and how they help reduce leakage.

3. Skin folds or abdominal contours

Abdominal shape plays an important role. Skin folds or scars may create small gaps that compromise the seal.

4. Inadequate skin preparation

Soap residue, moisture or oily skin can reduce adhesion. The skin should be clean and completely dry before applying the system.

5. Delayed appliance changes

Prolonged wear time may increase material fatigue and the risk of micro-leakage.

Practical prevention tips

  • Measure the stoma regularly, especially during the first months
  • Choose the appropriate type of baseplate based on body contour
  • Maintain healthy peristomal skin
  • Follow a consistent change schedule

When should you reassess your system?

  • Frequent or recurring leakage
  • Skin irritation or damage
  • Weight changes
  • Reduced sense of security in daily life

Changing your appliance does not mean failure. It means adapting to your body's needs.
