Up to 80% of people with a stoma experience issues with the skin surrounding it, but these are not inevitable — with the right care and awareness, you can keep your peristomal skin healthy and comfortable.
Common Peristomal Skin Issues & Practical Solutions
1. Redness (Erythema)
What it is: Surface redness often caused by improper fit or excessive appliance changes.
Solution: Measure the stoma accurately and adjust the cutter template. Support the skin gently when removing the pouch. Avoid changing the appliance more often than needed.
2. Maceration (Skin Softening Due to Moisture)
What it is: Persistent moisture breaks down the skin, increasing irritation risk.
Solution: Dry the area thoroughly before applying the pouch. Use barrier film or powder only temporarily; discontinue once the skin improves. 3. Irritant or Allergic Contact Dermatitis
What it is: Skin reactions from adhesives or hygiene products causing rash or itching.
Solution: Switch to hypoallergenic supplies. Cease using suspicious products and always do a patch test if needed. Consult a specialist if the reaction persists.
4. Fungal Infection (Yeast)
What it is: A warm, damp environment around the stoma encourages fungal overgrowth.
Solution: Keep skin dry, consider antifungal powders or sprays, and ensure proper appliance fit for prolonged wear.
5. Adhesive or Hair Follicle Damage (MARSI)
What it is: Skin stripping from adhesive removal or irritation from shaving.
Solution: Remove adhesive gently with a remover, and shave carefully if needed (or use electric razors).
Daily Prevention & Care Steps
- Clean gently using water or mild, pH-neutral cleanser.
- Dry thoroughly before applying the appliance.
- Ensure the appliance opening is cut just 1–2 mm larger than the stoma.
- Support the skin with your hand while removing the pouch.
- Inspect the skin every change for early signs of irritation.
- Use convex systems, paste, or rings for retracted stomas or skin folds.
- If any rash, blistering, or breakdown appears, seek professional advice.
